HSE Health Passport for People with Intellectual Disability accessing Healthcare Settings.
The HSE Health Passport is a communication tool designed to support people with Intellectual Disability express their needs when in a health care setting, it contains five sections, these are; All about me, Communication, Medical History, Looking after me & Keeping me safe and happy.
Additionally, the HSE Health Passport will assist healthcare staff to get to know all about the abilities and needs of people with an Intellectual Disability who come into contact with a Healthcare Setting enabling them to provide better safer care by providing reasonable adjustments before undertaking any assessment, examination or treatment of people with Intellectual Disability.
This app is a digital replica of the printed HSE Health Passport and has been built to provide easier access to the passport for people with intellectual disabilities.
The application consists of 3 sections: ‘Home’, ‘My Passport’ and ‘Guidelines’.
In the ‘My Passport’ section the user can fill in the information to the five sections provided and save it privately to their device by clicking the ‘Save My Passport’ button.
If the user wants to change information on their passport they select the ‘Change My Passport’ button which will allow them to make the required changes.
Once the changes are entered the user must select the ‘Save My Passport’ button to save the changes to their device.
All data is stored exclusively in local storage on the users device.
